What is Piedmont HealthCare Senior Care Advantage?
Piedmont HealthCare wants to help you reach your health and wellness goals. That’s why we developed a special program called Piedmont HealthCare Senior Care Advantage. This program is not a new Medicare Advantage plan. Rather, it is a collaborative approach to care delivery that focuses on preventive care and providing care coordination resources for individuals enrolled in participating Medicare Advantage plans. Through this program, the primary care physicians and care teams at Piedmont HealthCare are able to offer services that focus on your total health.
Access to Services
Piedmont HealthCare patients enrolled in a participating Medicare Advantage plan are able to access the following services through the Piedmont HealthCare Senior Care Advantage program:
- Annual Medicare wellness visit
- Cancer screenings and preventive care visits
- Coordinated care with other health professionals and services
- Referrals to specialists and other health professionals
- Patient outreach about upcoming or needed appointments
- Care team support for chronic conditions
Options For Your Needs
Under this program, you have the flexibility to choose one of the participating Medicare Advantage health plans that we and Piedmont HealthCare work with. We contract with most major Medicare Advantage plans like Aetna, BlueCross BlueShield of North Carolina, Cigna, Humana and UnitedHealthcare.
If you are already a Piedmont HealthCare patient enrolled in a Medicare Advantage plan with one of the participating insurance companies, you automatically have access to the Piedmont HealthCare Senior Care Advantage program. Please consult with your Piedmont HealthCare physician if you are unsure.
